DISCO XXX: Application Forms

We are not currently accepting applications for DISCO XXX. The deadline for application submission will be 1 May 2026.

Please download the certifications page (PDF) here first, BEFORE filling in the DISCO XXX application form. You will need a scanned copy of it to complete the application.

The application form for DISCO XXX is a Google Form, fillable on-line. It can be found here.

Note that this form may be revised after submission until midnight, Hawai‘i Standard Time, on 1 May 2026. If you are unable to fill in this form on-line, please download a copy of it here (Word .docx) and email the completed form to DISCO@soest.hawaii.edu no later than midnight, Hawai‘i Standard Time, on 1 May 2026.
